Client Enters Treatment

An individual begins outpatient treatment at New Era Mental Health — clinical assessment, diagnosis, treatment planning.

Wraparound Needs Assessed

The clinical team identifies barriers to treatment success — housing instability, transportation issues, lack of support.

Referral to Hope & Chance

The client is referred to Hope and Chance for wraparound support services — housing placement, RES assignment, transportation.

Integrated Support Begins

Hope and Chance surrounds the client with stability while they continue treatment — no barriers, just healing.

Coordinated Progress

Both teams communicate regularly to adjust support as the client's treatment progresses toward lasting recovery.
