Check-ins & AccountabilityRegular check-ins, encouragement, and accountability to keep recovery on track

Behavioral Health CoordinationCoordination of behavioral health and community-based services

Transition & Employment PlanningSupport with transition planning, employment readiness, and permanent housing search

Clinical Partnership

Hope & Chance works collaboratively with New Era Mental Health and other approved behavioral health providers to help residents remain connected to treatment and make measurable progress toward recovery, independence, and stable community living.

Participation is based on eligibility, available space, program engagement, and the ability to follow the safety and community standards of the home.

Adults actively pursuing recovery from substance use, mental health challenges, or co-occurring conditions who are engaged in an approved outpatient, IOP, mental health, or recovery-focused program.
